EI notebook needs evaluation comment fields.

Description

Each evaluation item in the EI notebook should have a text area in which staff may enter free form evaluation remarks/comments. The comments can be written to the ratings file, e.g.

01:32:00, Evaluation, CMS, "Message should include which lanes are closed"

Note: When entering a comment, type the message and then press the Enter key. The Enter key causes the message to be written to the log file.
