Remove comments & ranking bubbles from EInotebook (for May 2019)

Description

For the May 2019 training we aren't going to allow instructors to enter comments or rankings. Perhaps leave the code in place but commented out as we will implement it in the future.

Remove appropriate rows from html() function in Evaluation.js. Fixed in r397.
